29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  Location:  2804-2810 Mayer, St. Charles, MO  The employer failed to make frequent and regular inspections to ensure all workers were in compliance with company safety and health policies and OSHA standards.  On 4/17/2014 the workers were exposed to the following hazards:  a) An approximate 16-foot 8-inch fall  hazard as employees worked on the roof without the use of an adequate fall protection system and/or fall arrest system  b) An approximate 16-foot 8-inch fall hazard as the employees used a ladder to access the roof that did not extend the necessary 3 feet above the landing.   c) Employees failed to wear eye protection while operating nail gun(s) to perform roofing operations.  d) Employees climbed the portable extension ladder carrying bundle(s) of shingles to the roof, not maintaining 3-point contact.  e) A portable extension ladder set up to the porch area was less than 10 feet away from one of four residential service drops.
