This violation was observed on or about May 5, 2014 in the Machine Shop where employees were exposed to struck-by hazards when lifting material using a 5-ton Industrial Equipment Co. overhead hoist with a missing safety latch on the hoisting hook without removing it from service. b. This violation was observed on or about May 5, 2014 in the Machine Shop where employees were exposed to struck-by hazards when lifting material using a 1-ton Contrx Crane overhead hoist with a missing safety latch on the hoisting hook without removing it from service. c. This violation was observed on or about May 5, 2014 in the Machine Shop where employees were exposed to struck-by hazards when lifting material using a 15-ton Wright overhead hoist #1 with a damaged safety latch on the hoisting hook without removing it from service. d. This violation was observed on or about May 5, 2014 in the Machine Shop where employees were exposed to struck-by hazards when lifting material using a 15-ton Wright overhead hoist #2 with a missing safety latch on the hoisting hook without removing it from service. This violation occurred on or about May 5, 2014, in the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a Nicolas Correa, S.A. mill machine without interlocking doors. c. This violation occurred on or about May 5, 2014, in the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a Daewoo v65 CNC vertical mill machine without interlocking doors. d. This violation occurred on or about May 5, 2014, in the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a Leadwell LTC-50/cxxl CNC horizontal lathe machine without interlocking doors. j.
