29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a) At Lot # 183, Challedon Drive, Fairburn, GA- Employer was utilizing a wooden saw horse to gain access to the working platform of a ladder jack scaffold exposing the employees to fall hazards of approximately 6 feet 3 inches.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) House located at Lot # 183, Challedon Drive, Fairburn, GA: Employees using Pneumatic Hitachi Nail Guns, Model: NT 65M2 (S) and Model: NV 65AH were not protected from eye injuries while installing siding. Mario Antonio Cruz Osores was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.102(a)(1) which was contained in OSHA Inspection Number 947920, Citation #1, Item #2, issued on 12/13/2013 with respect to a work place located at 1483 Josh Valley Lane, Lawrenceville, GA 30043 and became a final order on 01/09/2014.
