29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which was used for the treatment of all injured employees, a person or persons were not adequately trained to render first aid: At the remote observatory facilities - Employees were not adequately trained to mitigate compoun fractures, amputations, or spinal cord injuries that can result from falls from heights or operation of heavy equipment in remote locations until emergency medical services arrive by motor vehicle located one to two hours away. Severe weather may prevent helicopter emergency medical services from arriving promptly at the remote locations.

29 CFR 1960.8(a): The head of each agency did not furnish to each employee employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm in that employees were exposed to the hazard of being struck by a John Deere 644E four wheel loader or caught between a John Deere 644E four wheel loader and another surface. At the observatory facilities - The employer did not ensure that a John Deere 644E four wheel loader and other four wheel loaders were maintained in accordance with the maintenance and operation manuals for the four wheel loaders. Among other methods, one feasible and acceptable method to correct this hazard is to follow maintenance procedures as delineated in the operation and maintenance manual for the John Deere 644 E four wheel loader and the operation and maintenance manuals for the other four wheel loaders onsite.
