29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) Worksite, Cement Mixing Area: Employee was assigned to change the bottom ring of W.E. Dunn Mixing machine (Model #HY10 / Serial #128) without being trained in the Lockout/Tagout procedures for the equipment; on or about 5/6/14. NOTE: In addition to abatement certification, the employer is required to provide abatement documentation for this item in accordance with 29 CFR 1903.19.
