An employee was observed standing on the railing of the basket while tying in wood for a concrete pillar.

29 CFR 1926.453(b)(2)(v): A full body harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about April 30, 2014, the employer failed to ensure that employees wore a fall protection when working in the basket of the extendable boom aerial lift. Two employees were observed worki8ng at a height of approximately 11 feet.
