29 CFR 1904.39(b)(1): Employer did not report a fatality or multiple hospitalization incident using the 800 number within 8 hours of discovery: a) The employer failed to report a fatality to either the Boise Area Office during office hours or the OSHA 800 number after hours. The incident occurred at approximately 6:00 P.M. on Saturday May 3, 2014, and the employer learned of it shortly thereafter when he reported it to the Twin Falls county sheriff dispatcher at 7:15 P.M. The employer notified the Boise Area Office Monday May 5, 2014 at 11:00 A. M. 41 hours later. Note: Abatement certification is Not required for this item
