29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. a) 2nd floor, west side: Employees used an electric powered screwdriver to do framing work. The screwdriver was connected to an extension cord, which was routed to another extension cord that is routed to an electric power source on the 1st floor. The immediate connected extension cord was missing a ground pin, and second connected extension cord did not have its ground pin plugged in. On or about 05/06/14. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

Midrails or equivalent intermediate structural members were not provided in the stairrail system. On or about 05/06/14. Note: Because abatement of this violation is already documented in the inspection case file, the employer need not submit certification of abatement for this violation as normally required by 29 CFR 1903.19.
