29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Southwest end of Main Shop, Keystone Avenue, Schuylkill Haven, PA - Employees cut metal pipe to size with a Gottfried Wahlen cold saw that did not have guarding protecting employees at the point of operation. Employees were exposed to a caught-in hazard, on or about May 7, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:  a) Southwest end of Main Shop, Keystone Avenue, Schuylkill Haven, PA - Employees were welding metals to form staircase handrails in the production area without noncombustible or flameproof screens or shields. Employees working in adjacent areas or walking past were not protected from the rays emitted during welding operations, as observed on or about May 7, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) Machine Shop, Keystone Avenue, Schuylkill Haven, PA - The employer had a written energy control program, but had not conducted a periodic inspection of the energy control procedures since August 1, 2011 for equipment, such as, but not limited to, ironworkers, presses, saws, shears, drills and grinders, identified in the program. Employees were exposed to caught-in, caught between and/or struck-by hazards in the event of minor changes or repairs to equipment having occurred, on or about May 7, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Employees were exposed to a struck-by hazard, on or about May 7, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
