29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: A. Automatic Fire Protection Systems Inc. worksite located at 501 W. Boundary St., Perrysburg, Ohio: On or about May 6, 2014, the employer did not ensure employees were adequately protected from cave-ins while installing a 12 inch waterline in a trench composed of "B" soil that varied in depth from 6.00 feet to 7.00 feet deep. Automatic Fire Protection Systems Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652 which was contained in OSHA inspection number 313783102, citation number 1, item number 2 and was affirmed as a final order on June 16, 2010 with respect to a workplace located at Findlay, Ohio.
