On or about April 24, 2014, the employer did not develop and implement written operating procedures that provided clear instructions for safely conducting temporary operations during work on the instrumentation system for the 6 CE York Compressor, and associated equipment, at the Rentech Nitrogen GP, LLC facility.    This item was corrected during inspection.

29 CFR 1910.119(f)(1)(iii)(E): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address any special or unique hazards:  On or about April 24, 2014, the employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities during work on the instrumentation system for the 6 CE York Compressor, and associated equipment, at the Rentech Nitrogen GP, LLC facility and did not address the special and unique hazard of an increase in nuisance or false alarms, as a result of the work, during operation.  This item was corrected during inspection.

29 CFR 1910.119(f)(1)(ii)(B): The employer did not develop and implement written operating procedures that provide clear instructions for steps required to correct or avoid deviation from the operating limits in the covered process.    Operating procedures, including but not limited to, the procedures for the following equipment at Rentech Nitrogen GP, LLC., did not completely define the operating limits and the steps to be taken to correct or avoid the deviation:  C-1 Air Compressor, C-11 Recompressor, B Clark Compressor, C-4 Refrigeration Compressor, C-4 Pressure Pump, Additional Clark Compressors, Purge Gas Compressor C-13, C-2D Syngas Ammonia Compressor, Chiller, HR-6 Startup Heater, AC-800 Sullair Air Compressor, Accumulator Pumps, T-1B Pump, and Crude Pumps.  The procedures did not specify the point at which operators were required to take or corrective action nor did it specify the appropriate action to be taken.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.119(i)(2)(ii): The pre-startup safety review did not confirm that prior to the introduction of highly hazardous chemicals to a process the safety, operating, maintenance, and emergency procedures were in place and were adequate:     On or about April 25, 2014, the pre-start up safety review for the new 6CE York Compressor and associated equipment at the Rentech Nitrogen GP, LLC. facility did not confirm that, prior to the introduction of ammonia to the system, safety procedures, including the labeling of the product lines, were in place and were adequate.      This item was corrected during inspection.
