29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  (a)  Throughout Facility - On or about May 2, 2014, the general Lockout/Tagout Procedure was developed in or around 1994 for the control of hazardous energy when performing servicing and maintenance on equipment and machinery, has not been inspected annually.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  (a)  Throughout Facility - On or about May 2, 2014, operators of equipment and machinery with hazardous energy sources were not provided with training to the affected level, in that they were not instructed in the purpose and use of the energy control procedure.   (b)  Throughout Facility - On or about May, 2014, all other employees whose work operations are or may be in an area where energy control procedures may be utilized, were not instructed about the procedure, and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out or tagged out.

29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) of 29 CFR 1910.333 and did not make it available for inspection by the Assistant Secretary of Labor and his or her authorized representatives:  (a)  Throughout Facility - On or about May 2, 2014, written procedures for working on or near exposed de-energized parts were not available, neither as a separate procedure or included within the written Energy Control Program.  ABATEMENT NOTES: 1.  The Written Procedures may be in the form of a copy of paragraph (b) of this section. 2.  Lockout and tagging procedures that comply with paragraphs (c) through (f) of 29 CFR 1910.147 will also be deemed to comply with paragraph (b)(2) of this section provided that:      a.  The procedures address the electrical safety hazards covered by this Subpart;      b.  The procedures also incorporate the requi4rements of paragraphs (b)(2)(III)(D) and (b)(2)(iv)(B) of this section.
