29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: 2050 Lincoln St., Blair, NE 68008 (Job-site): The employer is failing to ensure that a training program is provided to ensure that employees know to use adequate fall protection when they are performing residential roofing work on a house. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented: 2050 Lincoln St., Blair, NE 68008 (Job-site): The employer is failing to ensure that fall protection provided for employee use is used while employees are working on the roof of a home which is approximately 11 feet from a lower level. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed. Blanco Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 316017987, Citation number 1, Item number 1 and was affirmed as a final order on January 24, 2012, with respect to a workplace located at 2050 Lincoln St., Blair, NE 68008.
