29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: (a) On or about May 7, 2014 at worksite 10 Diary St Monett Missouri.- employees were exposed to serious struck-by and or crushing hazard, in that the finished goods storage shelving was not secured to the floor and was stacked with hundreds of pounds of cheese and supported solely by thin wire at the top post, exposing employees to a collapse. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

(b) On or about May 7, 2014 at worksite 116 Oak St, Monett Missouri.- employees were in the storage area were exposed to an electrical hazard when the power cords were affixed to metal conduit using zip ties. Abatement for this item is not required.
