On or about May 6, 2014, the employee working from the Genie lift did not have on a body belt and lanyard attached to the basket, exposing himself to a 30 feet fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about May 6, 2014, the employee walking out onto the balcony handing material to another employee in the Genie lift was not protected from falling 12 feet by the use of a guardrail system, safety net system, personal fall arrest system or any other type fall protection method.

29 CFR 1926.1053(b)(1): Where portable ladder(s) were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:  a. On or about May 6, 2014, employees were using a single rail ladder to gain access and egress from the next level.  The Werner ladder was not able to be extended 3 feet past the landing and was not secured to prevent accidental displacement.
