(a) Roof, Southside and Northside - On or about May 8, 2014 four employees were exposed to a fall hazard when performing sheathing installation without personal protective equipment (PPE) in place. There was only two personal fall arrest system on the jobsite and no safety glasses. The personal protective equipment was not provided by the employer.

(a) Roof, Southside - On or about May 8, 2014 two employees were exposed to being struck by flying debris or nails when using a hand-held circular saw and pneumatic air gun to cut and install plywood sheets and sheathing installation. Employees were not wearing safety glasses.
