29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, shall be guarded by a floor opening cover of standard strength and construction. While the cover was not in place, the trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings: a) In the pulley room a trapdoor floor opening was not protected on the exposed sides. Employees working at the site are exposed to fall hazard. The condition was noted on or about 05-06-14. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
