The fall hazard to the employees was approximately 12 feet to the ground.

Each employee on ramps, runways, and other walkways shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems. a) Jobsite, Lot 135 Monteverdi Drive, Oakdale, PA: On or about May 8, 2014, employees were walking on a 2 x 12 foot wooden plank to gain access into the residential house they were working inside of. The plank was used as a walkway over a 9 foot trench at the front of the house that had not been backfilled.
