29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  Jobsite, Lot 135 Monteverdi Drive, Oakdale, PA:  On or about May 8, 2014, two employees were putting paper down on a residential roof and were not protected from falling.  The fall hazard to the employees was approximately 12 feet to the ground.

29 CFR 1926.501(b)(6): "Ramps, runways, and other walkways." Each employee on ramps, runways, and other walkways shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems.  a) Jobsite, Lot 135 Monteverdi Drive, Oakdale, PA:  On or about May 8, 2014, employees were walking on a 2 x 12 foot wooden plank to gain access into the residential house they were working inside of.  The plank was used as a walkway over a 9 foot trench at the front of the house that had not been backfilled.

They told the csho they had been trained in fall protection, but did not have the written documentation to verify it.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Jobsite, Lot 135 Monteverdi Drive, Oakdale, PA:  On or about May 8, 2014, two employees were using a 24 foot Werner aluminum extension ladder to gain access to the roof, and it was not extended 3 feet above the landing surface.
