29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Jobsite, 2024 Croghan Drive, Carnegie, PA:  On or about May 8, 2014, two employees were flashing around skylights on a residential roof and were not protected from falling.  The fall hazard to the employees was approximately 21 feet to the concrete patio.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:    a) Jobsite, 2024 Croghan Drive, Carnegie, PA:  On or about May 8, 2014, two employees were working on a residential roof at heights of 21 feet above the ground, and were not utilizing fall protection.  The employer provided recent training records for the employees, but they did not demonstrate the understanding of the requirements or the knowledge of the hazards and how to protect against them.
