29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  This violation occurred on May 7, 2014 at a jobsite located at 3205 US Highway 75N Sherman, TX 75090; where the employer did not ensure that a Werner portable extension ladder side rails extended at least 3-feet above the upper landing surface. An employee was exposed to a fall hazard of approximately 15-feet.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  This violation occurred on May 7, 2014 at a jobsite located at 3205 US Highway 75N Sherman, TX 75090; where employer did not ensure that a Louisville A-frame ladder was used for the purpose it was designed for. The A-frame ladder was folded and used to gain access to an upper landing surface exposing employees to a fall hazard of approximately 23-feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  This violation occurred on May 7, 2014 at a jobsite located at 3205 US Highway 75N Sherman, TX 75090; were an employee stationing a ladder from a  ledge of a commercial building without fall protection was exposed to a fall hazard of approximately 15-feet.
