29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)     On the roof; an employee was exposed to a fall of 10' 8" from eve to concrete when working on the side wall flashing without benefit of any means of fall protection.  Note: Abatement certification IS required for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a)     At the eve; employees used the extension ladder which extended approximately 1 foot  above the eve line. 29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective was used:  a)    The Makita worm drive 7.5 inch skill saw with damaged cord was used exposing employees to a possible electrical shock hazard.
