29 CFR 1926.417(b): Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized:    (a) In power panel room 4:  There was no tag on circuit breaker number 5, and the circuit was not rendered inoperative as the breaker was not removed from the panel nor was the circuit wire removed from the breaker. The circuit supplied power to a single pole switch that controlled a light being installed in a hotel hallway. Employees that were installing 110 VAC light fixtures were exposed to the hazard of contacting electricity.
