29 CFR 1910.178(l)(4)(ii)(B): The operator has been involved in an accident or near-miss incident: On or about 04/04/2014, at 142 J S Mcdonnell Blvd, Hazelwood, MO, employee operating Nissan Forklift, Model #MAPL02A25LV, Serial #APL02-9U1233, travelling south along the east side of building 2, when the load(chip dumpster) slid off the forks. The employee has not received refresher training, as required by the standard.
