Employee was exposed to a fall hazard.

29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: 1) All Saints Angelical Church - Employees were working inside a Genie aerial lift truck approximately 40 feet high without a positioning device, which should have been attached, by lanyard, to either the basket or the boom of the lift while nailing timber board on the building. Employees were exposed to a fall hazard.
