On or about May 7, 2014 and at times prior thereto, the employer had not developed, implemented or maintained a written hazard communication program for employees who were exposed to corrosive chemicals including, but not limited to Acetic Acid and Glutaraldehyde.

On or about May 7, 2014 and at times prior thereto, personal protective equipment was not provided for employees who were exposed to chemical irritants including, but not limited to, Acetic Acid and Glutaraldehyde. Employee(s) were exposed to potential chemical burns of the skin and eyes.
