Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly: (a) Shop Area - Six foot long fork extensions were used on the Toyota forklift without the manufacturer's written approval, on or about May 9, 2014. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION STANDARD, OR IT'S EQUIVALENT STANDARD, 29 CFR 1910.178(A)(4) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 900231, CITATION NUMBER 1, ITEM NUMBER 5A, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 6, 2013, WITH RESPECT TO A WORKPLACE LOCATED AT 3711 BOARD ROAD, YORK, PENNSYLVANIA, 17406.
