29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    A) In bucket - An employee was exposed to head injuries while working in a bucket truck conducting tree branch removal/trimming activities. The employee was not protected by a hard hat or protective helmet, on or about May 6th, 2014.   B) Ground underneath bucket - An employee was exposed to head injuries while working below tree branch removal/trimming activities. The employee was not protected by a hard hat or protective helmet, on or about May 6th, 2014.
