29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) were not enclosed or guarded:  a) Production Area: On or about May 9, 2014, employees were using a Tannewitz vertical band saw with the non-working portion of the blade being inadequately guarded, exposing the operator to a potential laceration hazard.  b) Production Area: On or about May 9, 2014, employees were using a Powermatic vertical band saw with the non-working portion of the blade being inadequately guarded, exposing the operator to a potential laceration hazard.  c) Production Area: On or about May 9, 2014, employees were using a Delta vertical band saw with the non-working portion of the blade being inadequately guarded, exposing the operator to a potential laceration hazard.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) Front Mill Work Area: On or about May 9, 2014, an employee used a Black & Decker Heavy Duty bench grinder that had its tongue guard adjusted in excess of one fourth inch, exposing employees to injury from flying debris.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a) Front Mill Work Area: On or about May 9, 2014, an employee used a LAS III Volpato Horizontal Belt Sander which had a damaged electrical panel, containing parts energized to 220 volts AC, exposing employees to shock/electrical hazard from unguarded live parts.
