29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Ground below scaffold - Employees were exposed to head injuries while working below siding activities. The employees were not protected by hard hats or protective helmets, on or about May 7th, 2014.

29 CFR 1926.451(g)(1):   Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   A) Scaffold/Front of house - An employee was exposed to fall hazards of approximately 14 feet while working on a pump jack scaffold that was not protected by a complete guardrail system or other means of fall protection in place, on or about May 7th, 2014.    Paese Construction LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR.1926.451.(g).(1), which was contained in OSHA inspection number 313984908, citation number 2, item number 2 and was affirmed as a final order on March 15th, 2011, with respect to a workplace located at 22 Gloria Lane in Monroe, Nj.
