29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a)  Building 2, Web Department, Press #4:  Employees standing on gates were not protected from falling. The gates were located 49 inches above the deck plate, observed on or about 5/9/14.      Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:  a)  Building 2, Web Department, Press #4, Elevated platform: While employees are standing on the working platform located on the catwalk of the press they were not protected from falling approximately 7 feet to floor.  The existing hand-rail did not extend up to a height that would provide the required fall protection while working from that platform, observed on or about 5/22/14.  NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:    a) Building 2, Web Department, Press #4: Stairs were not provided to the gate platform of the Heidleberg Press.  The access plates that were provided measured 7 1/2 inches deep by 6 1/2 inches wide at the widest point.  The access plates were used several times a day. Observed on or about 5/9/14.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) Facility: The employer's procedures did not outline specific steps required by the standard for each specific type of equipment including but not limited to the Heidelberg Harris Press. The procedures did not include details such as but not limited to, how to shut down equipment, where to place the locks and tags for the specific energy of the machine, how to verify zero energy and the steps indicating how to restart the machine, occurred on or about 5/9/14.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
