29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used.  a) Front of house - On or about May 6, 2014, a badly worn extension cord was used to supply power to electrical hand tools.  The extension cord outer sheathing was torn in three locations and the internal wiring was exposed.
