Employees engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. a) Ocean Springs Hospital roof - on or about May 7, 2014, employees were working at the edge of a roof without using fall protection to prevent them from falling from the roof.

29 CFR 1926.602(c)(1)(vi): The all-terrain forklift did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. a) Ocean Springs Hospital - On or about May 7, 2014, an unsafe Genie GTH-644 all-terrain forklift was used to remove scrap material from the roof. The following safety discrepancies existed at the time of the inspection: 1. The rear tires were worn down to the tire carcass. The front tires were slick. 2. The horn located on the steering wheel was disconnected and removed.
