29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) were not protected by a cover that leaves no openings more than one inch wide. a) In the welding department employees were exposed to a floor hole approximately 11 inches is size where the Weld Line 7 exhaust ventilation ductwork went through the floor.
