29 CFR 1910.179(k)(1)(ii): The trip setting of hoist limit switches was not determined by tests with an empty hook traveling in increasing speeds up to the maximum speed: (a) Unit 3 Turbine Housing - On or about 5/7/14, employees were exposed to struck-by and crushing hazards when the limit switch was not tested to ensure proper working order. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
