On or about May 9, 2014, the scaffold platform erected between the first and second floor was not fully planked, exposing an employee to a 16 feet fall hazard.    b.  On or about May 9, 2014, the scaffold platform erected above the second floor was not fully planked, exposing an employee to a 26 feet fall hazard.
