29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely: a. On or about December 23, 2013, an employees involved in an incident involving a fork lift was not required to have refresher training prior to operation of materials handling equipment. b. On or about May 8, 2014, employees involved in minor incidents and near misses were not required to have refresher training.
