29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a) Shop area; The employer had not established or implemented a Lockout/Tagout program with machine specific procedures, training and annual inspections of the program to ensure that it was appropriate for protecting employees who perform service and/or maintenance on machinery or equipment, such as but not limited to press brakes, electrical panels, shearing machines, motors, milling machines, and drill presses.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Shop area, Cincinnati Press Brake (sn# 4672); The employer exposed employees to amputation hazards by allowing the use of a mechanical press brake which was not equipped with point of operation guarding.   b) Shop area, Accupress Press Brake (sn# 9713); The employer exposed employees to amputation hazards by allowing the use of a hydraulic press brake which was not equipped with point of operation guarding.   c) Maintenance area, American lathe; The American lathe machine was staged in the maintenance area and was not equipped with a guard around the rotating jaws, exposing employees to amputation hazards.   d) Maintenance area, Summit drill press: The drill press machine that was staged in the maintenance area was not equipped with a guard, exposing employees to amputation hazards.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles, to protect them from the rays when noncombustible or flameproof screens or shields were not used:  a) Shop area, welding stations; The employer exposed employees to welding arc rays by not installing protective screens within the welding area.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least:  a) Shop area, compressed gas cylinder storage; Employees were exposed to fire hazards as oxygen cylinders were stored within a foot of fuel gas cylinders, such as but not limited to, propane and acetylene.  The oxygen and fuel gas cylinders were found within the cylinder storage area.
