Broad and Olney Medical Center, Philadelphia, PA 19141 (Rear Storage Room) - An employee plugged the flexible power cord of a lamp fixture (Conserv-Energy BPCE13178, 13 watt, 120 VDC, 210 mA, UL #E170906) into a wall mounted electrical receptacle box. The employer did not ensure that the receptacle box was firmly secured to the wall exposing the employee to an electrical shock. Observed on or about May 8, 2014

Broad and Olney Medical Center, Philadelphia, PA 19141 (Rear Storage Room) - An employee plugged the flexible power cord of a lamp fixture (Conserv-Energy BPCE13178, 13 watt, 120 VDC, 210 mA, UL #E170906) into a wall mounted electrical receptacle box exposing the employee to electrical shock hazards as the employer ran the flexible power cord of this receptacle box through a hole in the ceiling. Observed on or about May 8, 2014

Broad and Olney Medical Center, Philadelphia, PA 19141 - The employees used Terumo Syringes with attached 25G X 5/8 inch needles (SS-01T2516) to administer Tuberculin Purified Protein Derivatives (PPD) to patients. These needles were not equipped with safety features designed either to eliminate or minimize the risk of accidental percutaneous injuries from needles contaminated with bloodborne pathogens such as Human Immunodeficiency Virus (HIV), Hepatitis B Virus (HBV), and Hepatitis C Virus (HCV). Observed on or about March 8, 2014.
