29 CFR 1910.335(a)(1): Employees working in areas where there were potential electrical hazards were not provided with, and did not use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed. Worksite: The employees, who were involved in removing hinged and bolted covers from 480 volt energized electrical panels were not provided with appropriate personal protective equipment. On or about 4/29/2014, an electrical arc flash explosion occurred while the employees were in the process of opening the covers of the electrical panels.
