29 CFR 1926.501(b)(1): An employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) On or about May 12, 2014, an employee was fatally injured from a 30-foot fall while installing plywood sheathing on the roof of a 3-story apartment building in the vicinity of 1270 27th Avenue NW in Minot, North Dakota.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about May 12, 2014, an employee sustained a 30-foot fall while installing plywood sheathing without the benefit of fall protection in the vicinity of 1270 27th Avenue NW in Minot, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 901885 , Citation 1, Item 1B, and was affirmed as a final order on 5/16/2013, with respect to a workplace located at 3882 Fourth Avenue East, Dickinson, North Dakota.
