29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a)BVB General Contractors at 10569 Chambers Road, Commerce City, CO 80022: On and before April 24th, 2014, the controlling employer did not ensure that the subcontractor employee conducting work in the sewer line excavation was protected from cave-in, in that the excavation was 7 feet deep and lacked an appropriate protective system. The working area was approximately 17 feet long, 8 feet wide and lacked any protective system, such as appropriate sloping, benching, or shield system. This condition exposed the employee to a crushing hazard by excavation wall cave-in.
