29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a). At Lot C-166 in Champions Gate where wood frame residential construction was being performed, the area was littered with debris which created puncture, slips, trips and fall hazards to the employees working in the area, on or about May 5, 2014.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. (a) At Lot C-166 in Champions Gate the 2o-foot extension ladder used to gain access onto the roof of the two story single family home under construction was not positioned at minimum of 3 feet above the landing and not secured at its top to protect the employees from the 10-foot fall hazard, on or about May 5, 2014.
