29 CFR 1926.501(b)(1): Each employee on a walking working surface (horizontal and vertical surface) with an unprotected side and edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) J & K Roofing at 1717 East 39th Ave., Denver, CO 80206: On and before 3/14/14, the employer did not ensure that fall protection systems were utilized by employees engaged in roofing activities while working at the unprotected side and edges of a roof on a commercial building. Employees were exposed to falls in excess of 20 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Because of the portable ladders length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in the mounting and dismounting the ladder: (a) J & K Roofing at 1717 East 39th Ave., Denver, CO 80206: On and before 3/14/14, the employer did not ensure that employee was utilizing portable ladders in such a way that enabled them to access and egress from the roof using ladders that extended 3 feet above the walking and working surface. Employee was using a portable extension-ladder for access to the roof of a commercial building. Employees were exposed to a fall of approximately 20 feet fall while attempting to use the ladder. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
