29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about March 18, 2014, at a jobsite located at 850 East Pioneer Irving, TX 75061; employees walking and working below workers above them did not wear head protection and were exposed to the hazards of falling tools or materials.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:     This violation occurred May 5, 2014, at a jobsite located at 850 East Pioneer Irving, TX 75061, where at least three (3) employees were performing roofing activities without fall protection systems. Employees were exposed to a fall hazard of approximately 11-feet.   STAR ROOFING & SHEET METAL WAS PREVIOUSLY CITED FOR A VIOLATION OF THE OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(10), WHICH IS CONTAINED IN:     OSHA INSPECTION NUMBER 314185430, CITATION NUMBER 1, ITEM 1, ISSUED ON 11/17/2011, WITH RESPECT TO A WORKPLACE LOCATED AT 3241 W. STORY RD., IRVING, TX 75062, AFFIRMED FINAL ORDER ON 12/15/2011;   OSHA INSPECTION NUMBER 314184375, CITATION NUMBER 1, ITEM 1, ISSUED ON 09/23/11, WITH RESPECT TO A WORKPLACE LOCATED AT 900 S. JUPITER RD. GARLAND, TX 75042, AFFIRMED FINAL ORDER ON 10/18/2011.
