29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) For employees working at 2931 West Hillsborough Avenue in Tampa, as observed on or about May 08, 2014. Employees were installing sheathing on a roof with a 5:12 pitch. The eave to ground height was approximately 10 feet and the employees had no fall protection system in place.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) For employees working at 2931 West Hillsborough Avenue in Tampa, as observed on or about May 08, 2014. Employees were on the second floor installing trusses and framework with an unprotected edge along the stairway to the first floor. The second floor to ground height was approximately 9 feet 6 inches and the employees had no fall protection system in place.

Employees were using a step ladder to access the upper working surface. The step ladder was closed and leaning against the outer wall and not used for the purpose it was designed. Step ladders are not designed to be used in the closed position for access to an upper landing surface.

Employees were on the roof beneath a suspended load of plywood sheathing being lifted by a crane without wearing any head protection.  Employees were exposed to an impact hazard.
