29 CFR 1926.62(n)(1)(i): The employer did not establish and maintain an accurate record of all monitoring and other data used in conducting employee exposure assessments as required in 29 CFR 1926.62(d): Employees were exposed to lead when cutting and installing lead-lined plywood panels for radiation protection in a medical facility and the employer did not maintain an accurate record of previous monitoring and objective data that included workplace conditions, work practices, control methods and other data used in conducting an exposure assessment.
