29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas are being used on the jobsite:  (a) 100 Congress St Springfield, MA: On or about April 25, 2014 the employer had more than 5 gallons of gasoline on the jobsite and a fire extinguisher was not provided within 50 ft. of where the gasoline was used to fuel an air compressor and a generator.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws:  100 Congress St Springfield, MA: On or about April 25, 2014, a yellow extension cord run from the generator to power a circular saw at the site had its outer insulating jacket pulled free from the attachment plug  allowing tension to be directly transmitted to the terminal screws of the attachment and plug.
