On or about May 15, 2014, employees working and walking in the Ice Bin Room had to traverse over loose catwalk metal planking and were exposed to slip, trip and fall hazard type injuries.

29 CFR 1910.23(e)(1): The railing did not have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level and the intermediate rail was not approximately halfway between the top rail and the floor, platform, runway, or ramp: a. On or about April 22, 2014, an employee walking to the Ice Bin Room was not protected by a properly constructed railing/guardrail system and was exposed to an 11-foot, 8-inch fall hazard. b. On or about May 13, 2014, an employee walking to the Ice Bin Room was not protected by a properly constructed railing/guardrail system and was exposed to an 11-foot, 8-inch fall hazard. c. On or about May 15, 2014, employees walking to the Ice Bin Room was not protected by a properly constructed railing/guardrail system and was exposed to an 11-foot, 8-inch fall hazard.

The Reddy Ice Corporation was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315481457, citation number 1, item number 2, and was affirmed as a final order on May 21, 2012, with respect to a workplace located at 4671 Edison Avenue, Jacksonville, Florida 32254. 29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a. On or about May, 13, 2014, in the loading dock and forklift battery charging area, the eyewash station was unsuitable for its intended purpose in that it could not provide quick drenching or flushing of the eyes if needed in case of a forklift battery acid release.
