Site - Employees performing machining work were not protected from amputation hazards when the rotating shafts and spindle ends of multiple bench mounted buffers were left unguarded. c. Site - Employees cutting fixtures were not protected from amputation hazards when the unused portions of the blade on a Kysor-Johnson horizontal band saw were not guarded.
