At the site located on Big Bend Road & US Hwy 301, Riverview, FL, employees involved in the installation of a manhole system were performing work inside a 12-ft deep and 20-ft wide excavation without a cave-in protection system. No adequate training had been provided in that no immediate action was taken to correct the created hazard, on or about, 05/13/2014.
